Mechanism

Paracetamol inhibits prostaglandin synthesis in the CNS, reducing pain and fever. NSAIDs (Ibuprofen/Aceclofenac) block COX enzymes, reducing inflammation. Serratiopeptidase breaks down inflammatory mediators, reducing swelling and promoting healing.

Pharmacokinetics

  • Absorption: Rapid oral absorption; peak plasma levels in 30–60 minutes.
  • Distribution: Widely distributed; crosses blood-brain barrier.
  • Metabolism: Paracetamol metabolized in liver (glucuronidation, sulfation).
  • Elimination: Renal excretion; half-life ~2–3 hours.

Side Effects

  • Nausea, vomiting, constipation
  • Gastric irritation (with NSAID combinations)
  • Rare hepatotoxicity (high-dose paracetamol)
  • Allergic reactions (rash, itching)

Contraindication

  • Hypersensitivity to paracetamol or NSAIDs
  • Severe liver impairment
  • Active peptic ulcer disease (NSAID combinations)
  • Bleeding disorders (NSAID combinations)

Interaction

  • Alcohol increases risk of liver toxicity with paracetamol.
  • Paracetamol may enhance bleeding risk with anticoagulants (warfarin).
  • NSAID combinations increase risk of GI irritation and ulcers.

Warning and Precautions

  • Use under medical supervision in patients with neuropathy or seizure disorders.
  • Monitor renal function during long-term therapy.
  • Avoid abrupt discontinuation to prevent rebound seizures.
  • Store at 25°C; excursions permitted to 15–30°C.
  • Keep out of reach of children.
